2017 Spring Club Activity Report Submission

 

1. Document for Club Activity Report : There are 3 sheets in the Club Activity Report Form

(1) Club Activity Report Form (Upload at Club board & Submit it in person)

(2) Sheets for Original receipts (Submit it in person)

(3) Sheets for pictures of activities (Upload at Club board & Submit it in person)

 

2. Contents of Club Activity Report Form 

(1) General Information

(2) Activity Overview: Please record your club activities.

(3) Application for Financial Support: Please record the information for the financial support. The information(Amount, Date..) is required to be same with your original receipts.

*Please upload at Club board & Submit it in person

 

3. Contents of sheets for original receipts

- It needs to be ORIGIANL RECEIPTS (not a delivery receipts, hand-written receipts..)

- Receipts need to be placed order of date

- Please attach it with glue (not a stapler, sticky tape..)

*Please submit it in person

 

4. Contents of sheets for pictures of activities

- Please attach as many photos as you can

*Please upload at Club board & Submit it in person
